At this time I only accept patients with work-related injuries and illnesses. I do not perform disability evaluations for non-work-related injuries or Social Security disability exams. If you need a Social Security disability exam contact your local Social Security office.  I also do not perform physical exams for pre-placement, DOT certification, or respirator clearance.

I accept workers compensation insurance, including federal insurance. I do not accept Medicare or Medicaid. I do not accept private insurance or auto insurance at this time.

Yes, we offer telemedicine appointments when appropriate for the needs for the patient.  We prefer that new patient appointments are in-person.

You will receive a link to create an account with our patient portal. There you will be able to complete the new patient questionnaire and provide more information about your workers compensation case.

You will be given a tablet when you arrive in clinic that will allow you to complete any necessary forms. We recommend that you arrive 15 minutes ahead of your scheduled appointment to allow time for this process.

You will be able to complete the necessary forms at your appointment.  Please arrive 15-20 minutes early to complete these forms.

Be prepared to answer questions about your date of injury, what happened when you were injured, what treatment you have received, and what other doctors and treating providers you have seen.  Complete this information to the best of your ability; the doctor will go over it with you at your appointment.  The form may take up to 15-20 minutes to complete.

Please arrive 15-20 minutes early for your first appointment to complete the registration process.  Please bring a photo ID (if you do not have a government issued ID, you may bring a work badge and/or documentation that confirms your address).  Please bring your insurance information and contact information for your case manager/QRC and/or attorney if you have one.

Yes.  If you have a QRC and/or interpreter they may attend the appointment with you; this is encouraged.  You can also come with one other adult such as a family member, caregiver, or support person.  This policy may be affected by COVID-19-related restrictions; if so, you will be notified when the appointment is scheduled.

We ask that you do not bring children to your appointment if at all possible.  We cannot provide child care during an appointment.

Please be aware that you are required to wear a mask in the clinic during your appointment due to current COVID-19 restrictions for medical facilities.  Please come to the appointment wearing a mask.  If you do not have one, a paper mask will be provided.

My office is located at The Workers Clinic, 7034 Brooklyn Blvd, Brooklyn Center MN  55429. Clients are seen by appointment only.

Yes, I will see clients for video visits when they are unable to travel to my office.
